To make your visit easier, AEHPC invites clients to complete and submit client and patient forms online.
We require this form for all new patients to gather essential information about your pet’s medical history, current condition, and contact details. Please fill it out upon arrival or submit it online in advance.
By completing this authorization, you approve the estimated costs for treatment. It allows us to proceed with emergency care after your consent is given.

This document allows you to specify your preferences regarding CPR and resuscitation in the event of an emergency. We will honor your decisions during critical care.

As is customary for emergency services, payment is due at the time care is provided. Emergency hospitals typically have higher fees than daytime general practices due to the specialized equipment, around-the-clock staffing, and advanced training required to provide 24/7 emergency care. If managing unexpected veterinary expenses is a concern, we encourage you to explore pet insurance options before an emergency occurs, as many plans offer a variety of coverage levels.
