Online Forms & Paying for Care

Online Forms

To make your visit easier, AEHPC invites clients to complete and submit client and patient forms online.

Admission Form

We require this form for all new patients to gather essential information about your pet’s medical history, current condition, and contact details. Please fill it out upon arrival or submit it online in advance.

Authorization Form

By completing this authorization, you approve the estimated costs for treatment. It allows us to proceed with emergency care after your consent is given.

Anesthesia Release Form

Before your pet undergoes anesthesia, this release provides consent for the procedure and acknowledges the risks involved. Please review and sign prior to treatment.

CPR/DNR Form

This document allows you to specify your preferences regarding CPR and resuscitation in the event of an emergency. We will honor your decisions during critical care.

Paying for Your Pet’s Emergency Care

We understand that emergency visits can be stressful, both emotionally and financially, and we are committed to clear, compassionate communication throughout your visit, always doing our best to find a solution to help.

As is customary for emergency services, payment is due at the time care is provided. In addition to the examination fee, if diagnostic testing or treatment is recommended, you will receive a treatment plan outlining the proposed services and associated costs. Our team will take the time to explain your options and answer any questions so you can make informed decisions that feel right for you and your pet.

Emergency hospitals typically have higher fees than daytime general practices due to the specialized equipment, around-the-clock staffing, and advanced training required to provide 24/7 emergency care. If managing unexpected veterinary expenses is a concern, we encourage you to explore pet insurance options before an emergency occurs, as many plans offer a variety of coverage levels.

For your convenience, we accept Visa, Mastercard, Discover, approved checks, and cash.

Financing options, including CareCredit and Scratchpay, are also available for qualified applicants.
